New user needs help

I am new to MYSql and just installed it for the first time a month ago. It is being used by a system monitoring app. Up until yesterday there have been no issues, then the following error started appearing and I cannot access the data.

This is running on a Windows 2003 server.

I just stared working with SQL and am not familiar witht he syntaxs
