Database design for dummie

Good day,

I'm a total idiot when it comes to mysql. I have to design a database that will get queries from a Borland c++ builder program. My questions is:

Firstly how 2 create a database.... that sounds stupid, but I can't get it working!
Secondly how to use it and access it on a remote server.

I can create tables with mysql, but that doesn't help without a place to put it in. lol.
