Well, here's my 2cents on this...
I tried everything above. Checked all client software... finally wound up looking at the hashing algorithm. Instead of shooting myself in the head... I decided I would try one last thing. (this is after doing everything else in this post...so you probably have to do the 'old password' thing as well.

I simply changed my password from 10 characters ...to 8 characters. Boom... I was in.
I'll work with changing it to more characters tomorrow... but basically... that was what did it for me.
