Can you make a simple test case to reproduce this problem?

The ideal case is a step-by-step set of instructions that will show the bug with a minimal amount of data.

You said that 6 users are sufficient for causing the problem, so it should not be a big deal to describe the steps to failing.
Therefore, please provide the following:
1) the proxy version and revision (possibly update to the latest one)
2) the server version
3) the command line statement used to start the proxy
4) for each user, their relative privileges
5) The structure of databases and tables that were involved. Ideally, one database only, with just a few tables, but if necessary, add as many as it takes.

With the above information, try to reproduce the problem using several sessions of the command line client.

To fix the problem, we need to be able to reproduce it. Thus, every simplification you can provide toward this end will help us to identify the issue.
