How do you run mysql-proxy?
Did you 'make install', or do you try to run it from the source directory?
The latter is a little complicated and not recommended, thus please to make install.

Please note, we usually have a wrapper-script that runs the actual mysql-proxy executable because it needs certain environment variables to find all of its stuff, like the Lua libraries and also LD_LIBRARY_PATH needs to be set correctly (/usr/local often isn't in it by default, that's probably why you see this error).
