Data not written to MYD on normal server stop

Hi,
I have installed mysql 4.0.23 on a dual processor box with Debian Linux / SMP and Raid.
Sometimes when inserting to a table the relevent MYD isn't being written to although querying the table shows the correct data rows. i.e. some in memory caching is being done.
Then following a normal /etc/init.d/mysql stop and then start, the data that was not actually written to the MYD is forever lost.
Any thoughts PLEASE
