I don't understand the "key 2" error. The MySQL 5.6 Windows commandline client chokes on the length of the column list, but when that problem's rectified by breaking the line, the INSERT works.
You're using an obsolete MySQL version. There may be probles with how Connector/ODBC 3.51 interacts with MySQL 4.1.
