MySql Problem

hi:
i'm sorry about i don't know where i can post this topic because i'm new in this field.

ok >> my problem is that i need to make a form in php language to upload a file <IMAGE> on a folder on my site by a simple form like:

as shown in the photo it will be a part of a form and i mean the MySql turn come here when i want to show the page with all it's inserted info. including the IMAGE that uploaded to the site

<about the upload button not required >> i mean it can upload when i click POST or SUBMIT>

please note :
1- I have MySql Ver. 2.2.3
2- I need replys becaue this situation is sooooo important for me
