Have exactly same problems on two machines running as slaves one ext4 other ext3.
So I think FS is not the reason.
Here is some output:

No chance to repair it!
My machines are
1) ext3, 2 * Quad-Core Intel(R) Xeon(R) CPU
E5320@1.86GHz running on Debian-Etch(!!) kernel 2.6.32 x86_64 GNU/Linux
2) ext4, 4* Sixcore, Intel(R) Xeon(R) CPU
X5660@2.80GHz running on Linux 2.6.32 trunk-amd64 #1 SMP Sat Dec 26 17:13:29 UTC 2009 x86_64 GNU/Linux
