My coworker found the problem. It didn't had to do with MySQL at all. I was storing token in a static variable and there's a case in the code, when a new token is not generated, but the variable is still used. So the token is used twice - that's why I thought there are two inserts, but no duplicate key error. Thanks Rick for taking your time.
