Most likely a server issue, check http://dev.mysql.com/doc/mysql/en/too-many-connections.html , http://dev.mysql.com/doc/mysql/en/out-of-memory.html and http://dev.mysql.com/doc/mysql/en/open-tables.html . Regarding the open files look at ulimit -a and the server config.
