I also noticed that the problem happened while a NDB backup was taking place. I have a cron job that runs the ndb backup twice daily, and this time around, 10 seconds into the backup the double failure was detected (and the backup failed, of course)
It ran many times prior with no problems, so the backup script itself is probably not the problem.
I have backups of clusterdata/ on each node just prior to my reinitialize/restore to get things running again. I attached the log from the mgm node to the bug report, and will be happy to provide any further information necessary to help find and fix the problem.
