How many rows in the production system> (I am assuming your test system has only a subset of the rows?)
SHOW TABLE STATUS
How much free disk space?
Is tmpdir on the main filesystem, or in a separate filesystem?
While the ALTER is running (and hanging), what is in SHOW PROCESSLIST and SHOW OPEN FILES?
