What is your mysql server doing at this point?
Run a SHOW FULL PROCESSLIST; and have a look in your error log.
2) 'Could not find first log file name in binary log index file' from master when reading data from binary log '
Have you ever moved files around/deleted files? What logs does the binary log index file list?
