I suspect the 'prefix' is preventing a sensible query plan. It seems to be scanning the whole table since it cannot trust the index to be in order. Yet another graphic example of why 'prefix indexes' should be avoided.

Did you time my other two suggestions?

Please use [ code ] and [ / code ] (without spaces) around things to preserve spacing.
