There is a limit somewhere of how big a row can be. (It might be 1GB.)
A BLOB fields has an extra 2 bytes for length; MEDIUMBLOB has 3. That is the only difference in how they are stored. Essentially no performance difference.
The ALTER TABLE will take time -- it will copy over the entire table, and rebuild all the indexes.
