Hi Tim,

Have a look at the following project listed at Freshmeat.net: (http://freshmeat.net/projects/phpdbform/)
I wonder if this is the kind of thing you might be talking about.

Also, there is Phil-a-form... see descrip. below (http://joomla-forms.co.uk/)
------
What is Phil-a-form?

Phil-A-Form allows Mambo admins to easily create dynamic forms using this component on their Mambo site.
The forms can be simple or complex and still no programming knowledge is needed!
Results can be emailed to anyone, or saved in a database - or one of many other actions!
All form elements are supported, along with form simple and complex validation rules
If you know no HTML dont panic - Phil-a-form will take care of that for you, however if you want the form to display using your own HTML then that is also possible, just use simple placeholders like {THIS} for your dynamic form elements!
-----

I've personally never used either but I've heard good things through the commnity over the years.
