If multiple users in multiple computers are to have access one remote db, you need either a secure setup on your own computer with a vpn through which remote users can connect to your app, or a web server on your computer, or a remote web server.
For 24/7 access the 1st 2 options require you to manage your server and keep it running 24/7. Most people find it cheaper & more convenient to buy hosting from a shared hosting provider. "Shared" hosting can be had for as little as $2-3/mo.
