Help desiging a new database

Hi,

I am trying to setup a new "Client Hair Colour" database for a friends Hair Salon. I have PHP / Apache and MySQL setup locally and all are running fine. I am able to use Dreamweaver MX to connect to a sample database and run queries with no problems.

My problem is that im not very good at desiging databases themselves.

Is there any websites I can go that have MySQL databases already built?

My needs for the database are pretty simple. All I need is a database that can hold clients info. e.g. (name, address, contact no etc...)with the ability to add information to each user as they return on their next visit.
