importing data dictionaries from xls or csv

I have data dictionary files that are currently in xls format. Is there an easy way to import these either by cl or phpmyadmin or Workbench?

The format is as follows:

c1, c2, c3, c4, c5, c6, c7
Column number, Column Name, Question Text, Response Code, Response Text, Data Type, Size

I've managed to be able to create tables via import by massaging a csv export into c2 headers for rows with a little awk, but would rather import it all as in the meta as well not just the Column Names.

I've figured out how to add meta comments via cl, does phpmyadmin or Workbench have an import for this?

# CREATE TABLE boo (
# id BIGINT UNSIGNED NOT NULL AUTO_INCREMENT COMMENT 'The KEY obviously',
