this is not a problem of the Migration Toolkit. Your MS SQL Server is refusing the connection.
"Network error IOException: Connection refused: connect"
Please search this forum, we did have very good response from people that have managed to configure the MS SQL Server to allow external access via TCP/IP over port 1433.
