I was having a very similar problem with MySQL 4.0.21 linux binary, running
on Linux kernel 2.6.8.1, while 4.0.20 had no such symptoms
If your setup is like mine, it may be that you are launching the mysql startup
script from the unix/linux root account and this account has TMPDIR = /root/tmp
defined. Unset TMPDIR and it may work out OK. Just twiddling the tmpdir
definition within /etc/my.cnf (with user = mysql, of course) does not fix it.
I don't know what, but something must have happened during 4.0.20 --> 4.0.21
and 4.1.3 --> 4.1.4.
