MySQL does not use indexes for unequality comparisons. And regarding the dtEntered field, is that of type datetime? If so, maybe the string comparison function gives MySQL a hard time, you could try using:

dtEntered BETWEEN '2006-04-26 00:00:00' AND '2006-04-26 23:59:59'
