I found these with a simple search:

These seem to cover most of the issues you mention. Perhaps I have more experience searching the docs, and we are working on ways to make finding the information easier.

As for phpmyadmin and PHP integration, we make it our task to document MySQL. We provide hints on using it with other tools where it seems prudent, but we do not consider it our responsibility to document the work of others.

And as for the CHM issues, you have us there. CHM is a new format and we are currently working on improving it.
