Many database indexes are ab+ trees; all databases at their core are NOT ab+ trees.

Trees pose special requirements, but you still need to normalise. See http://www.artfulsoftware.com/mysqlbook/sampler/mysqled1ch20.html for discussion with examples of various ways of modelling trees in MySQL.
