A decent modeling tool could be what you need although it may be a bit too much for you.

With a modeling tool you can edit your tables, columns, comments, etc and keep the database up to date from within the tool. The you can generate sophisticated reports for your documentation purpose. DBDesigner or ModelRight are free to use. Give them a try.
