Workers are often in a hurry punching in and out. In old but good punch card systems, there was no guesswork---the card told the system whether the worker was on her way in or out, and if the system detected consecutive ins or outs, it could flag an error for attention.

Beyond the usual rules of (i) requiring minimum input from the worker, (ii) requiring minimum guesswork from the software, and (iii) allowing maximum flexibility in the face of a high likelihood of future changing rules & circumstances, how you maintain accuracy depends on your system, see https://www.workforcehub.com/blog/best-employee-time-tracking-solution-replace-old-fashioned-punch-card-system/, esp. whether you have biometrics &c.
