Stesso problema da mesi... Le tabelle sono su Innodb e chiave autoincrement su un campo solo. Non potendo perdere dati, e usando la chiave autoincrement solo come servizio per la tabella, ogni volta che da errore vado a mano a 'liberare' il valore in conflitto tramite update e faccio ripartire la replica... :-(
