Si, le insert avvengono tramite stored procedure... Come dicevo in primary ho il solo campo auto_increment ma l'errore persiste... E' 'raro' come numerosità, ma fastidioso... Nel senso che avviene con una procedura complessa che macina qualche migliaio di record in un paio di minuti, e normalmente per ogni run mi capitano uno/due casi...
