You said you tried uninstalling MySQL and re-installing. Do you mean you tried uninstalling/re-installing the MySQL Server or MySQL for Excel?
You should try uninstalling/re-installing just MySQL for Excel (either from the MySQL Installer or from the Add/Remove Programs) while Excel is not running.
Make sure after uninstalling (and before re-installing) that the files at C:/Program Files (x86)/MySQL/MySQL For Excel 1.2.1/ are not present anymore and also that the registry key for MySQL for Excel at HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Office\Excel\Addins\ is also removed.
