Entity Data Model Wizard Unexpected Error

Been trying for days trying to figure out how to use Entity Framework on Visual C#.. I have used the different available Mysql Entity Framework packages from NuGet but either doesn't work or get the error when using the Entity Data Model Wizard.. The Output message is as follows:
