I think that this is a bug from MySQL and it happen in high volatile applications, with many changes for second. Could be a blocking from MySQL. If the SQL instructions are throwing more fast that they can be executed for MySQL, could be a possible error (buffer accumulation). This is a personal opinion. Are you using rollback transactions?
If you detect an SQL execute error from your code, don't modify your database, else they can be inusable. In this case, a REPAIR command help you sometimes.
