Ha... after pulling my hair out, too (what little there is of it left in the first place)... I found one additional cause of this problem, over & above what I've seen cited... you can get this dreaded "1005...150" error when you are trying to add a foreign key index whose name already exists in the database.

Remember: MySQL does does not maintain index names as specific to the table. They have to be specific throughout the database. You cannot have two InnoDB tables that in one database that have the same name for a foreign key index.

(I hope this advice saves some of you from the pain I've just suffered.)
