Giving the rows you want is most useful for ensuring that your programming still works properly when you add more columns. It's also faster to get just the rows you want instead of asking for all rows and discarding those you don't need.

There is already an index on the primary key so adding another index is unlikely to be helpful.

Are you using a my.cnf or on Windows a my.ini file? If not, using one will probably help. The buffer sizes you get with no my.cnf file are usually too small.
