The dev has added that index and the problem just happened again.
The reason I thought it could be hardware is because of how random it is. That query runs multiple times per day and always runs normally (under 15 seconds, usually only a few). But occasionally, it takes around 30 minutes. There is nothing else running, no higher load than usual (in fact it is often the only query at the time it starts).
When it happend one CPU core is 100% used with no IO wait and lots of free RAM.
