As I mentioned before index on (name,surname) is different. MySQL will only be able to use first keypart as you do prefix name matches. Surname should be more sellective, and if this is the only queries you use this index for you will only need (surname) as index.
There is command LOAD INDEX INTO CACHE command, it however does not work with FullText search index. You can however preload it in OS cache by simple cat "file.MYI" > /dev/null
