Problem of Disappearing User - Security Problem?

I have a server that only I have access to running a web application that handles databasing website visitor activity. Its been running fine for a year or so and then suddenly this morning the application gives me a mysql error 1044. I try logging in manually as the username and it refuses from command. I log in as admin and everything appears fine. Restart everything and still no luck.

To fix it I eventually just created the user/pass over again and it all has worked fine. My boss is worried about a security hole since the user just appeared to vanish. Does anyone know how a user would just vanish? Could it be some sort of attack?
