Yep, I checked the user table, that's how I knew something was wrong.
I know the user and host is the PK, but sure as xxxx there where two entries for root@% one with a password and one without.
There is either some issue with the grant statement or something funky with the PK constraint system. Under no circumstances should duplicates of a PK be allowed, yet it happened.
