I have found the pam_centrifydc.so was the main issue due to the credentials as you have already found. The solution was to remove the pam_centrifydc.so from the pam service file and replace it with the more standard pam_krb5.so module instead. It still uses the Kerberos standard (as implemented in Centrify), but does not have the crendential issues that you have seen so far.
