Thanks for the timings; it confirms what I suspected -- moving the DISTINCT into a subquery sped it up a lot (6x, in your case).

query_cache_size=256M -- too big. See the following for why, plus other recommendations:

I can't judge whether innodb_buffer_pool_size=14G is ok on your 54G box "with a lot of other apps". Probably it is now worth changing.

I suspect the summary tables will be the next big boost.
