Hi,

One thing I would recommend is upgrading to the most recent version (4.1.7 in your case) There are often lots of bugs fixed in newer versions that might be one you have hit upon. If you don't want to upgrade a production system, then you can still test on 4.1.7 to see if it still occurs.

If it still does occur then the bug system is at http://bugs.mysql.com

In order to assure the bug gets fixed, you need to have a reproducible test case. Just saying it doesn't work doesn't help all that much. Lots of other people use SET PASSWORD, so the developers need to figure out why it doesn't work for you. If you can reproduce the problem, then add the bug and include all of the information needed to reproduce the problem (including exact SQL statements run from a fresh install, dumps of data needed, etc...) That will assure that the bug will be able to be found and fixed.
