ERROR 1045 (28000) Access denied for user

Hi all, I'm an Oracle & MsSQL DBA, however on MySQL I'm very new in administrating it.

I notice some very very weird behavior on my current MySQL server. Intermittently my application guys will complain that they're getting Access denied error from their application. This same connection works some times without any changes. I've already granted all privileges as below.

+---------------+-----------+

Weirdly they're still getting intermittent Access Denied errors. Is there an existing bug on this MySQL version? (5.6.23 MySQL Community Server (GPL))
