If you have a problem like you just described it should be reported as a bug into MySQL bugs system:
https://bugs.mysql.com/report.php
However, please keep in mind that the diagnostic information you provided is relevant to the Microsoft SQL Server and does not help to identify a problem in MySQL ODBC Driver. You will need to give the specific instructions on how exactly to repeat this crash. Or at least upload a memory minidump from the crash.
