Fatal error encountered during command execution

Hello!
I have faced with the following issue in my app and still can't find any solution how to resolve it. Before creating this post I read a lot of different articles, but without any useful results. I try to describe shortly configuration: .net app + mysql connector version 5.2.5(pooling is on) + remote mysql server 5.0.84. The problems appears time to time while saving some binary data(blob) with the following exception(please, see below). After exception, one of the app's connections hangs on the server even the app is closed, moreover this connection blocks some resources and normal application work impossible even after restart. Any help appreciated.
