Storage engine? Is the insert code transactional?
How did you establish that the rows had actually been inserted?
Have you ruled out network failures &c?
Have you proved transactions weren't rolled back?
Is the app code well defended against sql injection attacks?
Does the error log have any entries relevant to this issue?
