Webform Widget Solution with Workflow

Hi, I am looking for some OpenSource or cheap technology that basically allows me to build forms from fields in a MySQL database, then package those up in a widget so the form can be embedded into multiple websites with speed.

The form can be quite complete, 3 steps, questions on the first step determine the products shown on the second step, they can select all or one and move to step 3 to provide details and committing the information to the database, similar to an eCommerce workflow.

Does anyone know of such technology I can use to speed-up my development process?
