embed dynamic pdf documents from MySQL into webpage

Hi

I am building a data entry application where I want on left part of webpage a embedded pdf documents retrieved from MySQl database. (the pdf documents is all loaded in MySQL). On right will be necessary form fields that need to be entered by looking at the information on left, pdf documents.

Each record has six page pdf documents.

How to get the pdf form element or how to embed or bound the field to display pdf docs. For example when i click next button i get next pdf documents from pdf stored in MySQL database. What tool is need in Php to do this. Can this be easier done in dreamweaver?
