Use phpMyAdmin 2.9 to connect to MySQL 4.1

Dear all,
I just setup a MySQL database on server and want to use phpMyAdmin to connect to it to do administrative task.
1. Is there any port on server need to be opened in firewall in order to let phpMyAdmin to connect? I need to open 3306, the default port, any others?
2. phpMyAdmin will use which port to connect to mysql? is it choose by random?
3. Do I need to first grant all privs to the account used by phpmyadmin?
