I was having the exact same problem on a windows xp server. I was using the installer version of php and it wasn't working... I could never get it to work right, so I did a manual install of php and did the extensions for mysql and it worked...

phpinfo() never would show mysql, and a reboot on my machine would show nothing, but after following the directions for a manual install of php 5, it all worked... and runs every time on restart...
