Connecting MySQL to PHP

I have done this dozens of times before at my alma mater and it's always worked perfectly. Now, when I try it on my own computer using a free web site that supports MySQL and PHP, I get error messages. I have a feeling it's because I don't know the right username and server to use. I tried finding the information in the manual, but no luck. What are the default username and server values? Or do I need to create a username? I'm guessing the server is 'localhost' (that's one piece of information I did manage to find). If you want to see the messages I'm getting, go here:
